What Is an Electrocardiogram?

Each heartbeat sends an electrical impulse that triggers the heart to pump blood. An ECG provides two major kinds of information: the time it takes for an electrical wave to pass through the heart and the amount of electrical activity passing through the heart's muscle, which can indicate if the heart is too large or overworked. Small, flat, sticky patches called electrodes are placed on the chest and limbs. These electrodes are connected by wires to an EKG machine, which picks up the electrical signals from different parts of your heart. The entire procedure most often takes just a few minutes and is painless.

How Electrocardiograms Are Used in Emergency Medicine

In emergency medicine, the ECG is an indispensable tool. It is often one of the first tests done on patients experiencing symptoms of heart attacks, like chest pain or shortness of breath. It can quickly assess the heart's electrical activity so doctors can determine whether a heart attack is occurring, the type of heart attack, and the best course of treatment. They can also help diagnose other conditions that might not be directly related to the heart but manifest through the cardiovascular system, such as electrolyte imbalances and some types of poisoning. 

How to Prepare for Your First ECG

Avoid using oily or greasy skin lotions and creams on the day of the test because they can prevent the electrodes from sticking properly. Wear a shirt that is easily removed to place the electrodes on the chest. If you're wearing stockings, you might need to remove them so electrodes can be placed on the legs. Also, be sure to inform your doctor about all the medications you take, including over-the-counter drugs and supplements. Some medications might affect the results of your EKG service, so your doctor may ask you to skip a dose prior to the test. However, this should only be done under medical advice.

Understanding the Results

The ECG generates a graph with lines that represent each electrical phase of your heartbeat. Here are a few key components typically analyzed:

  • P wave: Shows the electrical impulse starting in the atria.
  • QRS complex: Shows electrical impulses passing through the ventricles.
  • T wave: Shows the recovery phase of the ventricles.

Your cardiovascular doctor will look at these waves for rate, rhythm, and uniformity. Deviations from the norm can indicate potential heart issues that may need further investigation or management.

Common Abnormalities Found in Electrocardiograms

ECGs can reveal several types of heart abnormalities. Some common issues detected include:

  • Arrhythmias: These are irregular heartbeats. They can be too fast, too slow, or erratic.
  • Cardiomyopathy: This condition makes it hard for your heart to pump blood, which may lead to an enlarged heart.
  • Coronary artery disease: Blockages in the heart's arteries can reduce or block blood flow, which might be evident from the ECG.
  • Heart attacks: An ECG can show whether one of the heart's chambers is not getting enough blood.
  • Pericarditis: The inflammation of the sac that surrounds the heart, which can alter the ECG pattern.
